YOU ARE HEREBY SUMMONED AND REQUIRED TO APPEAR AND DEFEND in the above entitled action in this Court within the time period as required by the Arizona Rules of Civil Procedure. If you are served within Arizona, you are required to appear and defend within 20 days after service of this SUMMONS and COMPLAINT upon you, not counting the day of service; if process is served upon the Arizona Director of Insurance as the Insurer's Attorney to receive service of process against it in Arizona, said Insurer shall not be required to appear and defend until the expiration of 40 days after the date of such service upon the Director of Insurance.
YOU ARE HEREBY NOTIFIED that if you fail to appear and defend within the applicable time period, a Judgment by Default may be rendered against you for the relief demanded in the Complaint.
YOU ARE CAUTIONED, that in order to appear and defend, you must file an Answer or Response in writing with the Clerk of this Court, accompanied by the required filing fee, within the applicable time period; and you are required to serve a copy of such Answer of Response upon Plaintiff's Attorney whose name and address is Dennis P.
